The goal of the Saint Clare’s Health Geriatric Assessment Program is to work collaboratively with the senior, their physicians and family members in the management of the senior’s healthcare needs and chronic diseases. Saint Clare’s Health emergency departments are dedicated to providing emergency care tailored to meet the needs of the senior patient, while improving the overall experience for the patient and their family. The emergency team will work together to not only treat the patient’s current condition, but examine the patient as a whole and take into account other health factors that may affect the overall treatment and recovery of the patient.
