What Is the Geriatric Assessment Program?

The goal of the Saint Clare’s Health Geriatric Assessment Program is to work collaboratively with the seniors, their physicians, and family members in the management of the senior’s healthcare needs and chronic diseases. We also provide coordination of community resources when needed, and offer seniors a higher quality of life. Learn more about the GAP.

Developing a Plan of Care

Development of a comprehensive plan of care may include:

Addressing caregiver concerns and offer support
Chronic disease management and disease risk factors
Comprehensive exam and health risk assessment
Emphasizing the client’s functional abilities
Expansion of the geriatric care team
In-depth health education tailored to client’s specific needs

Medication management and education including indications and effects/side-effects
Nutritional needs
One-on-one consultation with a geriatric professional
Psychosocial concerns
Services required, and services received (e.g. PT, OT, VNA etc.)
