Overview
Saint Clare’s Transitions program provides a step-down from psychiatric inpatient care or an alternative to unnecessary psychiatric hospitalization for Morris County adults with a primary psychiatric disability or a co-occurring psychiatric and substance abuse disorder.

Our Approach
The multidisciplinary treatment team includes a psychiatrist, advanced practice nurse, licensed clinicians, and counselors. Programming emphasizes wellness and recovery and includes a variety of evidenced-based treatment options. Services are provided through one of three tracks: Acute Partial Hospital, Partial Hospital, or Intensive Outpatient. Treatment plans are individualized to meet the needs of each person.
Programs are available Monday through Friday from 9 am. to 3 pm.
